Use this form to apply for Safe Sanctuaries certification. To complete this form, you must answer self-disclosure questions.

Safe Sanctuaries - General Reference Form

This reference must be completed by a person who is not a relative of the applicant. The person providing the reference must have known the applicant for at least two years.

Safe Sanctuaries - National Background Release Form

This form grants consent to the Missouri Conference of the United Methodist Church to conduct an investigation into a Safe Sanctuaries applicant's background.

Safe Sanctuaries - Pastor Reference Form

This reference form must be completed by a pastor or a member of the program staff of a United Methodist Church. The person providing the reference may not be related to the applicant. If the pastor and the applicant are related, a District Superintendent or another pastor may complete the form.
